Where are these information?

I am trying to ask SSB to cancel my poursuit because it has been paid years ago. I was told to go online on the SBB site and ask this for the cancellation, but the site's form asks me these questions which I have no clue of.

I can't find those files. The pursuit I had was years ago, they somehow didn't cancel it even though I paid yet a long time ago. 
Now the only was to ask for cancellation is to fill and send these number, but the only number I have is what is written on my SBB SwissPass card. 

Where do I get these numbers?
