How to get a Swiss Pass?

Hi folks!

I'm moving to Switzerland permanently. I'm trying to sign up for a Half Fare Travel Card, but I'm getting stuck when I get to the Swiss Pass Customer number. From what I can tell, the Swiss Pass is sort of like you're profile and then you can buy different services that are attached to it (like the Half Fare Travel Card).

My question is this: How do I sign up for a Swiss pass initially? I see a form available online, but I'm not sure that I will have time to mail it to Switzerland and get it back to the US before I move. Can I go in person to get one? If so, where do I go and what will I need to do this (e.g., do I need to bring my own passport photo)?

I've done some searching online about where to go, but I keep getting information about 1-month tourist focused cards (which can be bought entirely online) rather than the 1 year resident focused cards. From what I can tell, it seems like most residents know what to do already...
